By submitting the form on this page, I understand that I am entering a working video production studio, and that certain risks are present, including but not limited to: low-hanging light fixtures, loose cables on floors or suspended from ceilings, a studio set facade that is not designed to support my weight and various other types of equipment utilized in video production. It is my responsibility to be aware of my surroundings and avoid these potential hazards.

By entering this studio, I agree to take full responsibility and hold harmless and indemnify Laser Stream Media, Inc., it’s owners, employees and contractors for any loss, accident or injury to myself, my employees, clients, contractors or anyone else who accompanies me at my request while on these premises. 

Furthermore, I agree to hold harmless and indemnify Laser Stream Media, Inc., it’s owners, employees and contractors from any actions at law resulting from video content created by Laser Stream Media or at Laser Stream Media’s studios that may result from final video content or claims made therein.

I also agree and understand that I am responsible for any damages to the studio, studio set, flooring, green screen or other parts of the Laser Stream Media facility at 1890 W. Oak Parkway, Suite B, Marietta, GA 30062.

This agreement serves as a blanket agreement for current and future visits, projects or rentals at Laser Stream Media, Inc.
